Partnering with Government Programs and Healthcare Providers

We pride ourselves on our collaborative approach, working hand in hand with various local Government Programs, including the Illinois Department of Human Services. Additionally, we have forged strong partnerships with Managed Care Organizations, aligning with esteemed healthcare providers such as Humana, Aetna, Blue Cross Blue Shield, Meridian, Molina, and County Care. These collaborations are integral to our commitment to the

state’s Home Services Program, ensuring comprehensive support and access to top-notch services for our clients. Rest assured, when you choose Healthcare Plus, you're not just getting exceptional
care – you're gaining a network of dedicated professionals committed to your well-being.

To qualify for the DHS Home Services Program, applicants must meet the following criteria:

• Hold U.S. citizenship or legal residency.
• Reside within the state of Illinois.
• Be under the age of 60 at the time of application for HSP services, unless applying for services under the HSP AIDS Medicaid Waiver or under the HSP Brain Injury Medicaid Waiver, in which case there is no age criteria.
• Have a severe disability that is expected to last for at least 12 months.
• Be an individual with a disability who needs long-term care, as determined by the DON score.
• Adult customers
(18 and over) must possess non-exempt assets valued at less than $17,500 (subject to exemptions)
• Minor Customers (under 18) may have no more than $35,000 in total family non-exempt assets.  To determine total family assets, the Customer and all other individuals who contribute to the family unit, or rely on the family unit for support, shall be counted.
• Be in the process of applying for Medicaid or already have Medicaid coverage.

Personalized Care Plans Tailored to You

At Healthcare Plus, we understand the importance of a seamless care coordination process. Upon the submission of applications, your dedicated DHS Case Manager from the respective counties promptly arrange appointments to assess eligibility. Once approval is granted, your DHS Case Manager  will  work diligently to develop a personalized care plan tailored to your unique needs. Through this process, they will allocate a specific number of service hours to each client, ensuring that you receive the level

of support necessary to enhance your quality of life. If you indicate your intention for Healthcare Plus to be your provider, your care will then be entrusted to us
